Adult Family Homes



Services

The home is responsible for basic and essential care; other responsibilities include safeguarding and promoting health, individual rights and the well-being of each individual resident. C of L Havens shall be compliant with all laws governing an Adult Family Home.

Other services that are provided include and are not limited to:

   The staff will, if needed arrange for resident transportation to appointments, religious services, or community activities. Transportation outside the use of facility/staff vehicles may be at the resident's expense.
   Monitor the health of residents and if needed set-up appointments for health services.
   Management, administration, and monitoring of presciption and/or over the counter medications.
   Laundry/Housekeeping Service - Normal and regular service provided. Special care cleaning of furniture, laundering, etc... may be addressed with home director. Residents may be required to help perform some of the household duties. This will be addressed in each Individual Service Plan (ISP).
   There will be three meals provided each day, including special medical and/or religious diets. Please note that there may need to be limitations set, please speak to the home director with concerns. A variety of snacks will be available. Resident's personal food will be labeled with name, date and properly stored. Expiration dates and/or standards of freshness will be monitored and maintained by staff. Food should not be stored in bedroom; there is a designated space in the kitchen for personal food items.
   Financial Management - Money - with written authorization the home director may hold or spend resident's funds. This authorization may be revoked at anytime with a written statement including an effective revocation date.
  • Resident funds will not be co-mingled with facility and/or other resident's funds. The home can retain no more then $200.00 petty cash per resident.
  • Keeping track of resident's petty cash - the home director will manage a legible, accurate accounting procedure. A record of all payments, disbursements and deposits made on behalf of the resident. The home director will provide resident or guardian with a written statement of account at least every six months.
  • Additional guidelines will be followed in accordance with state regulations.


Services not covered by monthly rate, and are not limited to:
  • Medications
  • Physician Services
  • Transportation costs outside the use of facility/staff vehicle.
  • Medical supplies and equipment.
Any questions, comments or concerns regarding supplies the C of L Havens will and/or will not provide should be addressed with the home director. Items may vary from resident to resident due to individual needs.


Programs

Activities and programming, which are provided by C of L Havens, will be at the level and frequency needed by each resident to provide needed stimulation. Activities will be chosen in accordance with resident capabilities and interests whenever possible. Activities and/or programming may be done individually with resident and staff or as a group with staff and home residents.

   Music / Aroma / Sensation / Stimulation Therapy
   Crafting
   Outdoor Activities
   Exercise
   Shopping
   Cooking
   Etc...
